Kindly print all registration forms (available online at www.hvrsd.org under Registrar/New Student Registration) and bring the completed forms to the district registrar, Beth Horvath, at the Central Administration Building, 425 S Main St., Pennington, NJ. Remember to include the legal documents listed below to verify proof of residency, along with your child’s birth certificate (with raised seal), and immunization and academic records available to you at this time. Our registrar’s office is open 8:00 am - 12:30 pm and 1:30 pm - 4:00 pm, Monday through Friday. Please call 609.737.4002 x2304 to schedule an appointment with the registrar to complete the registration process.

1. ONE of the following legal documents to verify residency in Hopewell Valley
   • Current tax bill
   • Valid driver's license with Hopewell Valley address
   • Deed or lease. Lease must carry signatures of lessor and lessee
   • HUD statement from closing (also known as a settlement sheet)
   • Sales contract with signatures of buyer and seller. Must include name and address or property location
   • Affidavit signed by a representative of a host family residing in the district, along with the host family's deed or lease
2. Birth certificate - original with raised seal or copy with raised seal.  Kindergarten and First Grade students only
3. Immunization records (copy)
4. Latest report card from former school (copy)
5. Latest testing scores from former school (copy)

10. Publicity consent form including Annual Notice of Parent’s and Student’s Rights Concerning a Student’s Records and Personal Information
